ONLINE REPORTS

As a convenience to Saginaw Township residents, the Saginaw Township Police Department has developed numerous reports that can be accessed and submitted from our website.

The forms are easy to complete and contain tips to help users along the way. Once completed, the forms are emailed to the appropriate department representative so that they will be on file. Some forms do require identifying information in order to be submitted. Other forms, specifically the ONLINE CRIME TIP form, can be submitted anonymously by the user.

Steps to completing forms:

1) chose the appropriate link.

2) read the general information at the top of the form to verify it is the form you wish to submit.

3) complete each data field paying attention to the ones marked as *required

4) submit the form

Once the form is completed and submitted, you should be taken to a confirmation page that will inform you of the successful form submission.


Forms:

REPORTBEAM ACCIDENT REPORT

The ReportBeam online accident report form is used for ALL crashes that require a State of Michigan UD-10 form. Once an electronic UD-10 is completed and submitted by the investigating officer it is immediately sent to ReportBeam.

Anyone involved in a crash on the roadways of Saginaw Township should receive a yellow sheet from the uniformed officer on scene. The yellow sheet contains the basic information needed to obtain a copy of the report. Users can follow the REPORT BEAM CRASH REPORT link and enter the corresponding information from the yellow sheet into the online form.

Please know that there is a small fee charged by ReportBeam. The fee is set by ReportBeam and cannot be waived by our department.

DELAYED/PRIVATE PROPERTY CRASH FORM

Sometimes a resident ma discover damage to their vehicle when returning to it after it has been parked and unattended. this most commonly happens at shopping centers or apartment parking areas. If there is no identifying information available for the vehicle or party that caused the damage, a DELAYED/PRIVATE PROPERTY CRASH FORM can be used to document the incident.

The form may also be used in times where a crash has taken place on private property and the parties involved have exchanged identifying information. Simply complete the DELAYED/PRIVATE PROPERTY CRASH FORM and submit it to our department via email. The incident will be reviewed and if it has been completed properly, a case number will be assigned.

A third circumstance in which the DELAYED/PRIVATE PROPERTY CRASH FORM can be used is for crashes that result in less than $1000 damage to the vehicles involved. Again, it is important that you have exchanged informed with the other party involved so that the report can be submitted properly.

HOUSE CHECK FORM

The house check form contains the same information that we ask when you call or stop into the office. If you are going to be away from your residence for an extended period and would like for the department to conduct a property check, please use this form. It is also important to notify the department when you return home so that the residence is removed from the property check list distributed to the officers. We have had several occasions in which an officer surprised a resident during a property check because the resident returned home early and failed to notify the department.

AT-RISK PERSON

The Saginaw Township Police Department offers family members and care-takers of persons deemed to be "at-risk" an opportunity to register the person. The AT-RISK PERSON FORM is easy to complete and allows the person completing the form to upload a photograph as well.

This report is most commonly used to provide information on Alzheimer's and dementia patients but it has also been used to register children that have Autism, are hearing impaired, blind or have other learning disabilities that affect their judgment of safe conditions. many of these children occasionally wander off from their home. We have had numerous reported incidents over the years in which a lost child that is learning disabled or unable to communicate has wandered away from home. Had the basic information and photograph been available to our responding officers, the time frame of safely returning the child home could have been greatly reduced.

The form is VOLUNTARY. It is simply a tool to help the process of returning a subject home to their loved ones. Residents that do not complete a registration for their loved one will receive the same professional service as would anyone in need.

LOST AND FOUND

The Saginaw Township Police Department is experimenting with an online form that residents may use to list minor items that have no significant value. Many time keys are misplaced while visiting our parks, shopping, or dining establishments. Residents may submit their lost to the department. The form will be reviewed and then listed.

Found property will be generically listed as well. Key identifying details will not be provided but a general area in which it was found and general item description will also be provided (see sample below)

FOUND:

ITEM #: 2010-0004 January 3, 2010. Keys with key ring. Parking lot of Fashion Square Mall.

LOST:

ITEM #2010-00001 January 1, 2010. Brown wallet. State St. near Center Rd.

ONLINE CRIME TIP

Perhaps our most valuable online report form, the ONLINE CRIME TIP form allows residents to offer information on suspected criminal activity or current investigations. Residents are given an opportunity to provide information to the investigations division without having to leave their name.

The form does allow users an opportunity to be contacted by the department if they chose to do so but it is not necessary to give that information in order to have the matter investigated.
